Patient Info/Forms

Check out our easy-to-follow steps to get started at Skills on the Hill. Whether you are a first-time parent seeking support or a seasoned caregiver expanding your services – we are here for you every step of the way. Contact us directly with any questions or comments on taking the first step!

  • For new patients or existing patients who wish to add a service, please complete the New Patient Contact Form below. Please make sure to provide all information requested and upload relevant documents that will help us understand your child’s therapy history. If you have any difficulty with this form, feel free to call us at either location and we can walk you through the process.
  • Once the form is submitted, we will verify your insurance benefits. This may take up to 3 days for some insurance due to our need to verify the information by phone.
  • Look out for an email titled “Please fill out this form” in your inbox. It will include your FINANCIAL AGREEMENT, INSURANCE INFORMATION, & ESTIMATE FOR SERVICES. This document must be thoroughly read and reviewed, credit card information must be provided for us to keep on file, and the agreement must be signed in order to schedule services.
  • After you review your insurance benefits and sign your Financial Agreement, please look for an invitation to our Patient Portal where you will need to complete our Intake Paperwork and Consent Forms. This information must be completed in order to schedule an appointment.
  • The last step involves us contacting you to schedule an appointment. If your child has received therapy services in the past from another provider and does not require an updated evaluation, he or she will be scheduled into a weekly therapy slot. If a comprehensive evaluation is required, then we will offer you available options for scheduling an initial evaluation appointment.

Directions:

The address for the Arlington office is 2301 Columbia Pike, Arlington, VA 22204. The parking garage is located under the building and the entrance is in the back, in an alley off of South Wayne St (across from Bob & Edith’s Diner) right after the Dry Cleaner. Follow the signs for public parking to the entrance and turn down the ramp. After you pull a ticket from the machine, look for Row A first then Row E in the garage to park (you will make 4 left turns forming a square). Bring the parking ticket up with you; we can validate your ticket when you come up for your session. Make sure to take the elevator from the parking garage – not the stairs, they stay locked – to floor M. The stairs are marked in green and the elevator is marked in grey. When you get to M, turn right. You will come to the door of suite 125 and check in. Our staff will perform a temperature check and

The address for the DC office is 1301 Pennsylvania Ave SE WDC 20003. There is free/unzoned 2-hour street parking on adjacent streets and metered parking in front of the building. Our entrance, which is handicap accessible, is to the back of the building. When you arrive to the door, please use the callbox and someone will buzz you in.

  • Please arrive on time for your child’s scheduled appointment. If you have not arrived within 10 minutes, we will give you a call.  Any late cancellations or missed appointments will incur a “late cancel/no-show” fee as indicated in the signed Financial Agreement.
  • Following the evaluation, the therapist will provide a very brief summary. You will receive a report and individualized treatment planning will be discussed via phone at a later time or at the start of the next session.
  • If therapy is recommended, you will plan a series of regularly occurring sessions with our office scheduler. The number of treatment sessions required will be determined by the recommendation of our therapist and will be discussed with you at your first appointment.
  • If your child is transitioning to Skills on the Hill from another practice, the therapist will develop a treatment plan after approximately 3-4 sessions to get an accurate picture of strengths and areas to address in therapy.
  • As therapy progresses, our therapist will track progress regularly.  Re-evaluations tend to occur around 6 months following the evaluation, unless insurance requests a re-evaluation sooner to show necessity for continued treatment.

Please do not hesitate to contact us at any time if you have any questions. We want to make this process as easy and comfortable as possible for your child!
